If your product is under warranty, include the proof of purchase. Do not provide payment information. 
If your product is out of warranty, provide the following payment information.

Print and include this completed form in the box when shipping your product.

If out of warranty, you will be charged a non-refundable bench fee of $140. 
The $140 bench fee includes a complete diagnosis and is applied to the repair cost. 
If the total repair cost exceeds $249, we will contact you prior to repair for approval. 
If the total repair cost is $249 or less (excluding shipping), we will repair your product and you will be charged the balance when it is shipped back to you.
